REINTEGRATION
PROGRAM

 
  Persons eligible for reintegration are those who are inappropriately living in nursing homes who wish to return to the community. The Reintegration Program assists those who lack the resources or home care needed to live independently. WCICIL may assist disabled persons aged 18-59 who meet the income guidelines to qualify for this program. We offer help with the initial costs for rent, furniture, appliances, utilities, handicap accessibility and services from a personal assistant or other home health care for customers needs. WCICIL also offers other services to help make the transition a success.

The philosophy of Independent Living does not allow for people with disabilities to be forced into nursing homes, when they can receive the care they need at home. WCICIL is dedicated to empowering people with disabilities to choose the setting in which they live.

Who is eligible?
Any person who:
 

bulletCurrently resides in a nursing home
bulletIs between the ages of 18-59
bulletHas a disability, and
bulletLives in Adams, Pike, Brown, Schuyler, Hancock, or McDonough Counties. Referrals can also be made to other Centers for Independent Living that serve other communities throughout the state.

How can the Reintegration Program
and WCICIL assist you to find independence?
 

bulletAssist with the transition from a nursing home to independent living at home.
bulletAssist with paperwork.
bulletHelp with locating accessible housing.
bulletAssist in finding appropriate personal assistants or nursing care.
bulletSupply necessary household items.
bulletensure all assistive devices are available and in place.
bulletAssist with establishing utility hook-ups, rent, and housing deposit.
bulletProvide for minor home modifications, when necessary.
